Net 30 days following date of invoice. A finance charge of 1-1/2% per month, which is an annual percentage rate of 18 percent, will be charged on all past due accounts. Title to equipment or goods covered by this invoice shall remain with Organic Authority  until paid for in full. Purchaser agrees to pay Organic Authority  collection costs and reasonable attorney fees if this becomes necessary. I authorize release of credit information to Organic Authority LLC.

I authorize Organic Authority  to set up a Customer Account. I attest that the information given above is correct and that if any business information changes Organic Authority  will be notified within 14 days.
